[FFmpeg-cvslog] fftools/ffmpeg_filter: add filtergraph private data

fftools/ffmpeg_filter: add filtergraph private data

Start by moving OutputStream.filtered_frame to it, which really belongs
to the filtergraph rather than the output stream.
