iam glad we agree here, this is very sensitive stuff and its very easy to
create a huge mess, the very first step is to understand what really is
needed then we must think of how to do that in a way which allows
all the currently working use cases like remuxing without decoder and
remuxing between the same container and different containers ...
for the palette case it seems like a palette flag in AVPacket will be
needed and AVPacket will have to be passed as argument to avcodec_decode_video
for TOP_FIELD_FIRST it seems the situation is not fully understood,
i doubt that the flag is needed for decoding mjpeg at all

there is extradata and the packets themselfs any other general flag
passing will completely break remuxing between formats which have a
unequal set of such flags
