Have a look at
http://roundup.mplayerhq.hu/roundup/ffmpeg/issue481
it might be the same issue. I posted a patch that might be able to fix 
the problem you are experiencing:
http://roundup.mplayerhq.hu/roundup/ffmpeg/msg2250
but such patch seems to create problems on windows, so it has not been 
committed to svn.

Anyway, you can try it to see if it helps in your setup.
