Here's a new version of the patch.  I've fixed, or so I hope,
everything people have pointed out, as well as some errors I found all
on my own.

I'd appreciate if Windows and Darwin users could give it a try.  I
fear I may have overlooked something in those areas as I cannot test
it on these systems myself.
