The patch file indeed has some problems...
What I do is applying the patch, then copy all dvbmuxer/mpeg* to
libavformat. Most of times it works. :)

The TS muxer currently does not handle its buffers correctly, and the
resulting file is still not MPEG compliant - I'm not sure its even
better than the current muxer.
