Both patches look correct to me and tested, they fix (at least) issues
2359 and 2378.

I have further minor fixes (AV_LOG_INFO stuff), but can be done as a
separate patch.

Note: APIChanges need to be updated.

Note to the tester: when updating the source,
tools/lavfi-showfiltfmts.o is not updated, and the resulting
lavfi-showfiltfmts crashes, resulting in no test output. I resorted to
manually remove tools/lavfi-showfiltfmts.o.
