I have to correct myself.  When you said intra refresh -- I though 
intra-only coded files.  Of course those are two different things.  
Intra refresh does not require IDR slices, but it does require SEI.  So, 
this broken mkv file could have used intra refresh, but I don't think it 
does because SEIs aren't there.
