You will have to add the missing information as 
explained on http://ffmpeg.org/bugreports.html
Without it, there simply is no way (except wild 
guessing) that anybody can help you.
If you did not compile yourself (and cannot test 
with a debug build), you will have to report at 
the place where you downloaded the binary.
(The FFmpeg project only offers source code, no 
"products".)
