The trouble with that is, this happens for me with every file I've
tested, and can be reproduced with a simple 'mplayer <filename>' - even
the -fs switch is not necessary, it happens the same way if I toggle to
fullscreen afterwards.

I just tried with '-noconfig all', and the exact same thing happened.

Since I happen to have the 'WobblyWindowsIntro.ogg' file from the
samples archive already on hand, I just tested with that using the
command line

/tmp/mplayer/mplayer -noconfig all WobblyWindowsIntro.ogg

(that being the 'brand-new SVN checkout' copy of MPlayer I built in
response to your request), and on going fullscreen it was shifted in
exactly the same way.

I can provide any degree of verbose output desired, in the unlikely
event that that would help.

I'll plan to try updating my NVIDIA X drivers after work today and see
if that changes anything. Can you confirm that your own tests which did
not show the problem were using 180.29, or if not, what version they
were using?
