[MPlayer-dev-eng] [PATCH] dvb-input menu and more fixes

Hi,
sorry to pester you with another patch, but this time I removed all the 
hacks and added a menu of channels.

It's for MPlayer-20030505.

What's changed:

- compliant with the new stream api and syntax (including dvb://prog)
- removed all hacks that I'm aware of (e.g. video_id and audio_id)
- removed many useless options
- removed the need to close and reopen the stream when changing channel
- removed all globals. Functions now work almost always on private data
- added a menu of channels (tested with OSD)
- re-enabled support for DVB-C cards
- added autodetection of tuner-type (SAT, TER, CBL)
- added a command: dvb_set_channel used when selecting a channel via menu
- added dvbin-howto.txt in DOCS/en
- improved the speed when changing channel (doesn't tune if not necessary).
