I was going to comment on this patch but you already committed it..

IIUC the patch then the window is always in the foreground now.  All
X11 video output drivers (except SDL - anybody feel like sending a
patch?) toggle foreground operation with the -ontop switch and the "T"
key.  IMO vo_quartz should honor that as well.
