I tried -pre6 of your patch, and it fixes the weird xv issues I had.
(The thread I started about this is "Strange XV behavior", but nobody
replied.)  What I'd like to know now is why this patch fixed the xv
problems.  :)

Looking at the diff, I can't see the particular change or bit of code
that fixed this.  Any hints for me?

I'm sure the weird xv business can be fixed independantly of the
fullscreen stuff.
