Actually, I've found DVDs with broken sync. (Missing some audio or
video frames? It's strange because PTS never resets, but "ct:" jumps
periodically while playing the file with mplayer...) So I'm retracting
the proposal to make mc=0 the default, for now.

But, I do want to fix this massive brokenness. Anyone have ideas how
to make mc!=0 work right? Lots of the code is horribly unreadable...
