[MPlayer-dev-eng] Small ao_jack patch

Hi, I will be doing a bit of tweaking to the Jack output layer shortly 
(now that my system is pretty much Jackified now!).  First a quick patch 
to default the volume levels when we startup. (Otherwise it all starts 
rather quietly)

Patch to allow opening of a default device to follow shortly:
