I made that change and several others. Updated patch attached.

But unfortunately it looks like the whole switch_audio thing
is going to be useful only for some audio formats, because
libfaad for example chokes on the switch and outputs garbage
for up to a couple of seconds.

Not even AVI? There are plenty of dual AVIs out there.
Oggs too, but demux_ogg is a pretty bad mess, so it's gonna
be tough.
