Ok. But without special handling for dvd stream, general solution 
(at demuxer.c) isn't possible: order of the streams can be changed 
due to stream type or user preferences. However, such approach has 
its advantages: not every format supports several 
audio/video/subtitle tracks.

I changed only those demuxers, which formats allow multiple tracks, 
but track info is not provided by them. I intentionally left 
MPEG2/MPG demuxers unchanged, as they probably may be used while 
playing DVDs, thus tracks info duplicating is possible and likely 
it can not be avoided without special casing for dvd streams.

Changes affect avi, asf, mov, real, lavf and nut demuxers. They are 
very similar, so come on in single patch.
