There's a separate MIME header, Content-Disposition, with standard
values "inline" and "attachment" telling whether the mail reader should
by default present the message part inlined or as a separate attachment.
Ulion's previous mail with a patch did have this header with an explicit
value "attachment", so mail readers should not default to showing it
inline whatever the content-type is.

If you want an attached patch to be shown by default you should set
Content-Disposition to "inline". How to do that depends on the program
of course; in Evolution there's a "Suggest automatic display of
attachment" checkbox in the attachment properties.
