[MPlayer-dev-eng] rtmpe support

I don't know if there's even any need for this, since things are working with 
ffmpeg. I just didn't want this code to be lost if anyone wanted it. Seeking 
is still broken, it has some wrong interaction with the cache manager that I 
haven't figured out. So, this may serve as a model for other protocol 
implementors, or it may just be a historical footnote.
