[MPlayer-dev-eng] [PATCH] Remove ivtv ao and vo drivers

Hi,
Just got the mail on today, the private one never reached me out (or get
spammed, whatever ...).
I think that time has come years ago already for IVTV support to get to and
end.
So feel free to get it removed.
