with Xv drivers, I don't know about professional cards)
which can do multiple overlays at the same time.

so you can't do it, it's a hardware limitation!
one window will work, multiple won't.
you should try -vo x11 or gl instead of xv

you shouldn't use 0.17a... it's very old & buggy.
try 0.18pre3

i think it's problem with your audio card driver.
try 0.18pre...

impossible. 4.0.3 has no built-in Xv driver for nvidia cards.
