This sounds just like a bug with b-frames I reported:

This was fixed in libavcodec CVS a couple days before rc4, so I don't
know if it was included. Try the latest 0_90 CVS. The instructions are
on the mplayer download page, but substitute "0_90" for "main". Make
sure you retrieve CVS libavcodec too; the instructions are here:
