That patch fixed a problem playing DVDs, and didn't affect their
encoding. It sounds like you have just one disk that is messed up this
way. If you make an mplayer -dumpstream of enough of the DVD to show
this problem and upload it to ftp.mplayerhq.hu (read
DOCS/bugreports.html) the developers might be able to see the problem.

Meanwhile, does this option help for you (from the manpage):

-audio-delay <0.0-...>
      Sets the audio delay field in the header.  Default is 0.0, negative
      values  do not  work.   This  does not delay the audio while
      encoding, but the player will see the default audio delay, sparing
      you the use of the -delay option.
