I used 4.2 then. I upgraded to 4.3 some weeks ago, and changed my card
about the same time, so I can't test it now, sorry.

It happened with exact video combinations and fixed-vo. With the random
music videos encoded with different resolutions and parameters, about
10-20% of video transitions gave me this effect.

There is even more evil fixed-vo bug: when using -vo x11 and video
switches while watching in fullscreen, mplayer suddently goes to window
mode and then ignores 'f' key, which makes fixed-vo unusable with x11
driver...
