Very often the movie is the first title on the disk, which is exactly what 
gets played by using mplayer dvd:// (why it doesn't play all chapters is a 
mystery to me as well). The disk that doesn't work probably has the movie on 
one of the other titles. You can have a look at all the titles by using 
mplayer dvd://2, mplayer dvd://3, mplayer dvd://4, etc.
