mm, old card, slow cpu. only a couple things you can try
1. try mplayer -vfm ffmpeg -lavdopts -lowres=1 dvd://1
or lowres=2 or maybe even 3, this REQUIRES a recent CVS compile of mplayer
2. use -framedrop and -cache

dont have to install X to try it, head to http://movix.sf.net and try 
out movix2 (bootable cd with X and MPlayer...)

xv is the better video output, also xvidix when it works resonably fast.

like others have said, only XVMC supports hardware mpeg2 decoding
and i think its only on certain cards, like nvidia. more hardware support is in
the http://mplayerhq.hu/MPlayer/DOCS/tech/wishlist .
