I'm not sure if there is an option to do this, but you can insert a line 
of the format
delay: [sign]hh:mm:ss:ms
before the first timestamp: line in the .idx file. 
It seems to be important that all the numbers are at least two figures (ms 
can also be three), otherwise the line takes no effect.
