This is the bit I'm trying to figure out how to do :-)
Thanks to RC, I've found

which talks about 2pass encoding for mpeg4, so I may be able to
triangulate and get it done for mpeg2/dvd.  Rest assured when I get
the beast working I'll post something here :-)
