Reach over and smack the shift button every couple of minutes.
Otherwise, you're out of luck. Mac OS X support isn't nearly the
level you'd get for Linux support (though it is quite good,
especially compared to other OSS projects). Anyway, I don't think
MPlayer understands Aqua yet. The Linux command to stop the
screensaver (which must be invoked manually) works only for
XScreenSaver. If you're using another screensaver app (which is
unlikely, if you're using Linux) then MPlayer doesn't know what to
do. That's kind of your situation with OS X.

I suppose if you could rig up a script to simulate a key press every
couple of minutes, that might work. As a totally cross-platform
option, perhaps someone should create a small box that hooks into
your USB port and simulates a USB mouse or keyboard, and, while
turned on, sends the system a command to move the mouse by a pixel,
or press some innocent and (independently pressed) functionless key,
like shift, every 1, 2, 3, 4, or 5 minutes (possibly selectable by a
switch?). Probably won't happen, but it's worth throwing out there.
