there's nothing to configure: you just have to run mplayer with
-mouse-movements to enable the item selection by means of
the mouse, or just leave it always set in your config file: 
~/.mplayer/config

mouse-movements=1

As for the rest dvdnav:// will accept -alang and -slang like dvd://
