FWIW, i have mplayer working with external dvdnav, dvdread and dvdcss.
I use :
  --enable-dvdnav
  --enable-dvdread
  --disable-dvdread-internal
  --disable-libdvdcss-internal
  --with-dvdnav-config=/path/to/dvdnav-config
  --with-dvdread-config=/path/to/dvdread-config

just my 2 cents.
