This is missing lots of information.
What operating system?
What exactly happens when MPlayer cannot play the file?
What is the full MPlayer output?
What happens with -noidx or -forceidx?
Also MPlayer and ffplay behaving the same way is highly suspicious, they use completely different code to read AVI files, unless you configured MPlayer differently.
