Every vo that it matters for needs to implement it.  This is because it
involves the windowing system, for which there is little abstraction.
There are plenty of vo's that should be ignoring this anyway.  I say
implement it on a per vo basis and add CTRL_ONTOP or whatever it was to
the tech docs about vo design.  That way, new vo's that can use it will
probably have it written from the start.

Hmm... how should -ontop behave for fbdev in console?  Maybe without it,
we use background console color as a colorkey and let console text be on
top.  :)
