Solved for DivX files. This is a hack, I'm reading some bits of the
divx frame headers to determine which frames are keyframes...

Also a new option has been added:  -forceidx
it forces rebuilding idx even if file has idx chunk.
Useful for testing, and when teh file has broken/bad index.

DivFix fixes keyframes for divx, but it make non-divx files unusable.
