I use XFree 4.0.2 and xv worked right 'out of the box',
i.e. without _any_ options in XF86config, only

Section "Device"
    Identifier  "Intel i810"

If it does not work for you, maybe i810 cannot do xv.

Your X11 shows 
(II) Loading extension XVideo
right ?
