Sorry I'm stumped. I couldn't see anything wrong with your output.
Does anything else work with xv, like xine? Does X load the v4l
module? What if you restart X? Have you tried rebooting? Are you
running NVidia's binary driver or just X. You need at least X 4.2.0 for
xv without the binary driver.
