"mplayer tv://1 -vf rotate=1" works fine for me in linux. That is with 
hardware scaling. Whether it will drop frames or not will depend on your 
system. On my XP1800+ rotated fullscreen TV eats about 35% with XV 
(576x768), without rotating it takes about 29-30% (768x576)
