hi there,

your problem is most likely a saturated codec.
you should try what happens when you use a fixed quanitzer of 2.
a fixed quant of 2 is what you get when the codec is saturated, meaning, it
is not able to compress "lighter" and has reached its maximum quality.
actually a good thing.
you could benefit from this and start using a higher resolution, use less
noise-filtering (if you use such at all..), raise your audio-bitrate and so
on..

in you initial post though, you stated that the quality was very poor.
investigate what causes that.
in my experience lavc has big trouble with noisy sources, where xvid is able
to keep the noise intact, using higher bitrates. xvid seems to be more
capable of "detecting" the noise as fine detail, where lavc seems to simply
ignore it as it possibly can't deal with such fine structures.
