I just realized that there was a nasty bug in write_mpeg_pes_header():
the block that reads:

if(h->dts)
{
      write_mpeg_ts(&buff[len], h->dts, 0x1); // 0001 before DTS
       len += 5;
 }

must be:

if(h->dts)
{
      write_mpeg_ts(&buff[len], h->dts, 0x10); // 0001 before DTS
      len += 5;
}
