vhq / mbd are overlaping options if you will.  Leave vhq out.

Your output file is named ".mp4".  No matter what you name it, mencoder
is producing an AVI file, not an mp4 file.

You should start with a simple command-line...  Start without the *cmp
options, and without most of the -vf options, and then add options, one
at a time (and try various values), to see what improves the picture,
and what doesn't.  Nobody can just tell you what the perfect
mencoder options (for all purposes) are.
