So

mplayer -vo xv -fs [video]

Only gives you a video in a small portion of the screen? If so I'm
guessing it might be dropping back to x11 mode in that case try

mplayer -vo xv,x11 -fs -zoom [video]


And for documentation 'man mplayer' has it all.
