Thanks Carl, even that helps me restructure my question..

It seams that given this information, I should recompile mplayer and
disable all external decoders (such as mad audio decoder). As far as
I know I don't have any old files. Also, based on the output of
Knoppix based mplayer, it appears that mplayer plays the file using
only ffmpeg codecs.

The file plays without error when tested with Knoppix (live CD)
"MPlayer svn r34540 (Debian), built with gcc-4.7"

My next question is, when I issue the command "mplayer -vc help" does
mplayer query the ffmpeg's codec library to generate the list of
available codecs? Is the utility mediainfo from
http://mediaarea.net/en/MediaInfo useful in determining if ffmpeg
supports the required codecs? Mediainfo does return video and audio
codec id numbers. Last question is there on on-line list of which
codecs are supported by each version of ffmpeg?
