Sorry for breaking the thread with my last post! I hope it's okay how I 
do it now!

I meanwhile installed MPlayer SVN-r31895-4.4.3 based on this guide:

But I've the same problem with that version. Things I tried:
different graphics card (NVidia Geforce/Ion/Quadro, ATI 38xx & 4850)
different OS (Ubuntu Lucid, Mandriva 1 SpringED)
different PCs (Ion-based, AMD and Intel 4-Cores).
different video output driver like vdpau, xv...
I tried to compile mplayer by taking the tcsetattr(...)
out of /osdep/getch2.c and adding an additional output,
which doesn't improve the situation and just showed that it
happens after [[getch2 uninit]] output. There's no difference
between different kind of videos (mpeg/mpeg2/h264).

The delay happens between
[[[uninit getch2]]]
Config poped level=3

Here's a shorted mplayer output, the beginning of video 1 was created by 
using level 5, the interesting part is level 9:
